Referral
Process
Adapted
physical
education
services
are
available
only
to
those
students
who
qualify
for
placement
in
special
education
programs.
Adapted
physical
education
is
physical
education
which
has
been
adapted
or
modified
so
that
it
is
as
appropriate
for
the
person
with
a
disability
as
it
is
for
a
person
without
a
disability.
Students
with
disabilities
are
entitled
to
the
same
number
of
hours
of
physical
education
as
their
nondisabled
peers.
Process
If
case
managers
are
in
question
as
to
whether
or
not
a
child
qualifies
for
adapted
physical
education,
they
need
to
inform
the
adapted
physical
educator
to
allow
for
an
informal
observation
time.
If
determined
by
the
adapted
physical
educator
that
the
child
may
qualify…the
adapted
physical
educator
will
speak
with
the
case
manager
and
school
psychologist.
It
is
the
duty
of
the
case
manager
and
psychologist
to
mail
consent
forms
and
then
to
schedule
meetings
once
the
evaluation
has
taken
place.
This
is
a
team
process.
Adapted
Physical
Education
is
an
individualized
program
involving
physical
and
motor
fitness,
fundamental
motor
skills
and
patterns,
skills
in
aquatics
and
dance,
and
individual
and
group
games
and
sports
designed
to
meet
the
unique
needs
of
individuals.
Eligibility
‐
the
following
criteria
are
considered
in
determining
a
need
for
APE
services.
•
Score
of
85
and
below
on
the
Test
of
Gross
Motor
Development
‐
2
•
Exhibit
a
developmental
delay
of
2
or
more
years
based
on
appropriate
assessment
instruments
•
Function
within
the
severe
or
profound
range
as
determined
by
specific
eligibility
standards
•
Possess
social/emotional/behavioral
or
physical
capabilities
that
would
render
it
difficult
for
the
student
to
reach
his
or
her
physical
education
goals
without
significant
modifications
or
exclusion
from
the
general
physical
education
class.
Referral
Process
•
Complete
an
Adapted
Physical
Education
Referral
Form—obtained
from
school
psychologists
•
Send
the
referral
to
the
APE
teacher
and
school
psychologist
•
The
APE
teacher
will
conduct
an
informal
observation
to
determine
if
evaluation
is
needed
•
If
evaluation
is
needed….a
consent
to
evaluate
is
required
•
Upon
completion
of
evaluation
pertinent
team
members
will
meet
with
parents
to
discuss
results
and
placement
options
We
currently
have
two
areas
under
the
Adapted
Physical
Education
umbrella.
If
a
child
does
not
qualify
for
APE
but
does
have
some
gross
motor
issues
it
may
be
suggested
that
they
participate
in
Modified
physical
education.
A
modified
curriculum
may
occur
in
either
the
general
education
setting
and/or
adapted
physical
education
setting
with
the
goal
as
placement
in
the
general
physical
education
classroom.
Consultation
is
available
for
a
minimum
of
six
months
at
which
point
in
time
the
general
physical
educator
may
email
or
call
the
adapted
physical
educator
for
ideas
and
information
as
needed.
During
that
six
months
the
adapted
physical
educator
will
put
the
consultation
on
his/her
rotating
schedule
to
meet
with
the
general
physical
educator
regularly.
The
idea
is
that
the
Adapted
physical
educator
will
provide
enough
assistance
and
ideas
that
the
general
physical
educator
can
continue
the
modified
program.
